What to Expect at Your Appointment
What to Expect on Your First Appointment.
Your care and treatment outcomes are our highest priority. Your initial appointment will be your longest to ensure that we collect all the information that is essential to make accurate diagnostic assessments and optimize your treatment planning.
Step 1: Completing Digital Paperwork
First you will receive digital paperwork to fill out regarding your treatment history, this paperwork is brief and includes a pick list of options so that you can move onto your next step without repetition and redundancy.
Step 2: Meeting with an Intake Coordinator
1-2 days before your appointment with the provider you will meet with our Intake Coordinator. They will review your intake paperwork and ask some additional questions, collect some baseline clinical scales, and gather any information to record the severity of your symptoms.
Step 3: Meeting Your SMART Providers
Team evaluation with your SMART providers who will be working together to dive deep into your symptoms, provide a diagnostic assessment and discuss your treatment plan with you.
Step 4: Meeting Your Care Team
After you meet with your providers, you will be meeting with our Care team who will complete any tests or studies ordered by your physician. Your Care Navigator will also assist you with planning for your care and providing you with any education about the next steps of your treatment plan.
Step 5: Completing Any Testing or Lab Work
Your Care Navigator and Care Team will assist you in setting up additional diagnostic testing your provider recommends prior to your next appointment.
Step 6: Second Appointment Review
When you book your first appointment we will also book you for your second appointment. Your physician will review all assessments recommended after your first appointment and collaborate with you on your tailored treatment plan and next steps.

Team-Based Evaluation and Ongoing Care
At SMART Health & Wellness Centers, we believe that understanding the root cause is as important as treating the symptoms. Our team-based evaluation process is unique to our model.
Before your first appointment, you will receive a call from our licensed intake coordinator, who will provide a quick phone consultation. They will make sure to gather any important information needed for your evaluation and answer any questions you may have.
At your first appointment, you will be seen by both the nurse practitioner and psychiatrist you will be working with. Our nurse practitioners collaborate and work closely with their overseeing physician throughout your evaluation appointment and any ongoing care appointments. Expect to interact with both your nurse practitioner and the overseeing psychiatrist for your ongoing care appointments.
How to Prepare to Meet Your Intake Coordinator
We ask that you have your medications with you (or present if telehealth) so that we can accurately record them.
If you have had any labs completed in the past 30 days, please have copies ready.
If you have been discharged from a psychiatric facility within the past 60 days, we ask that you have the following:
