Your First Appointment

What to Expect

Your first appointment is a detailed look into your medical history, emotional health, and personal goals. We use this visit to build your personalized treatment plan and coordinate care with you and Care Team.

Step 1: Completing Digital Forms

First, you’ll receive digital intake forms by email to complete your treatment history and background information. We ask that you please complete your intake forms at least 48 hours before your visit so our team can prepare and ensure a seamless experience.

 

 

Step 3: Second Appointment Review

When you schedule your first appointment, we’ll also reserve your second visit. During this follow-up, your physician will review the results of any assessments from your initial appointment and collaborate with you to finalize your personalized treatment plan and next steps.

Step 2: Meeting Your Care Team

After checking in with our front desk, you’ll begin your evaluation with your Collaborative Care Team, who work together to understand your symptoms, complete diagnostic assessments, and discuss your treatment plan with you. Your Care Navigator will guide you through next steps and providing education about your treatment options.

Step 4: Testing & Lab Work

Your Collaborative Care Team will help schedule any additional testing or lab work your provider recommends before your next appointment, ensuring a smooth and coordinated care experience.

Step 5: Review Treatment Options

After your provider reviews your test results and treatment plan, you’ll meet with your Care Navigator to discuss your treatment options, receive education about your care, and coordinate or schedule your next steps.

At each of your following appointments, your provider will determine if your appointments need to be in person or may be performed via telehealth.  Our scheduling team will work with you to accommodate what times and dates work best with your schedule.
